Roughly half of men between 40 and 70 have some degree of erectile dysfunction, and a growing share of them are being offered a 5 mg tadalafil tablet every morning instead of a 20 mg tablet on date night. The daily version behaves less like an event drug and more like a vascular medication, which is why it has become a fixture in men's health clinics alongside testosterone, GLP-1 drugs and statins. This guide covers what the trials actually show, where the heart data is stronger than most men realise, and where the marketing runs ahead of the evidence.
Daily tadalafil 5 mg keeps a PDE5 inhibitor in the blood around the clock, which changes it from a drug a man takes for an evening into one that acts on his arteries, his prostate and his pelvic blood flow every day. The trials show a 7 to 9 point improvement in erectile function scores, a 4 to 6 point drop in prostate symptom scores, and measurably better artery function within 4 weeks. This guide separates the effects with solid randomized evidence from the ones that rest on observational data, and lays out how tadalafil fits with testosterone therapy.
Daily tadalafil for men over 40 has become one of the most common prescriptions written in men's health clinics, and it is worth understanding why the daily version exists at all. Sildenafil and the original 20 mg tadalafil tablet were built to be taken before sex. The 5 mg daily tablet, approved by the FDA for erectile dysfunction in 2008 and for enlarged prostate symptoms in 2011, was built to hold a constant low level of the drug in the blood, so that the arteries in the penis, the prostate and the rest of the body are exposed to it continuously. That shift in dosing changes what the drug does, who benefits and what the risks are.
This guide walks through how tadalafil differs from the other PDE5 inhibitors, why erectile dysfunction at 45 is a cardiovascular warning rather than a bedroom problem, what the randomized trials found for daily versus on-demand dosing, the prostate data, the blood vessel and heart outcome studies, what tadalafil does and does not do to testosterone and muscle, who must never take it, how it combines with testosterone therapy, and a 12 week plan for starting it properly.
17.5 hours
Half-Life Of Tadalafil, Versus About 4 Hours For Sildenafil
Tadalafil stays active for up to 36 hours after a single dose, and at 5 mg a day it reaches a steady blood level after about 5 days. Sildenafil and vardenafil clear within 8 to 10 hours, which is why they were never developed as daily drugs

Improvement In IIEF Erectile Function Score On Daily Tadalafil Versus Placebo
In the Porst 2006 and Rajfer 2007 randomized trials, tadalafil 5 mg once daily for 12 weeks raised the IIEF erectile function domain by roughly 7 to 9 points more than placebo, comparable to on-demand 20 mg, with 60 to 70 percent of intercourse attempts successful versus about 30 percent on placebo
4 to 6 points
Drop In International Prostate Symptom Score On Tadalafil 5 mg Daily
Across the four registration trials pooled by Porst in 2011, tadalafil 5 mg reduced the IPSS by 4.8 to 6.3 points from baseline versus 2.2 to 3.6 on placebo, an effect similar to the alpha blocker tamsulosin 0.4 mg and reached within 4 weeks

Share Of Men Aged 40 To 70 With Some Degree Of Erectile Dysfunction
The Massachusetts Male Aging Study of 1,290 men found 52 percent reported minimal, moderate or complete erectile dysfunction, with complete dysfunction rising from 5 percent at 40 to 15 percent at 70
About 3 years
Average Lead Time Between New Erectile Dysfunction And A Coronary Event
Montorsi and colleagues found that in men who developed both, erectile dysfunction preceded coronary symptoms by 2 to 5 years, a median of about 3, because the 1 to 2 mm penile arteries narrow before the 3 to 4 mm coronary arteries do

Mortality After A First Heart Attack In Swedish Men Prescribed A PDE5 Inhibitor
Andersson 2017 in Heart followed 43,145 men after a first myocardial infarction; the 7 percent who filled PDE5 inhibitor prescriptions had 33 percent lower all-cause mortality and 40 percent fewer heart failure admissions. Observational, so partly a healthy-user effect, but consistent across several registries
Daily tadalafil differs from on-demand tadalafil in dose and in blood level: a 2.5 or 5 mg tablet taken every morning produces a steady concentration of the drug around the clock, while a 10 or 20 mg tablet taken before sex produces a peak two hours later that fades over the next day and a half. All PDE5 inhibitors work the same way. Sexual stimulation makes the nerves in the penis release nitric oxide, which raises a messenger called cyclic GMP inside the smooth muscle of the arteries, which relaxes them and lets blood in. Phosphodiesterase type 5 is the enzyme that breaks cyclic GMP down. Blocking it lets the signal last longer and go further. The drug does nothing without the stimulus, which is why no PDE5 inhibitor causes an erection on its own.
Tadalafil is the only one of the four approved PDE5 inhibitors with a half-life long enough to make daily dosing practical. Sildenafil clears in about 4 hours, vardenafil in 4 to 5 and avanafil in about 5. Tadalafil clears in 17.5 hours, so a 5 mg tablet every day accumulates to a steady level after roughly five doses and stays there. The practical consequences are that spontaneity returns, because the man is never waiting for a tablet to work; that the drug reaches the prostate and the systemic arteries every day rather than a few times a month; and that the dose is low enough that the classic side effects, the headache and flushing that come with a 20 mg peak, are much less common.
The Four Approved PDE5 Inhibitors Compared For A Man Over 40
| Drug | Onset And Half-Life | Usual Doses | Notes For Men Over 40 |
|---|---|---|---|
| Tadalafil (Cialis, generic since 2018) | 30 to 45 minutes; half-life 17.5 hours, active up to 36 hours | 2.5 to 5 mg daily, or 10 to 20 mg as needed | The only option for daily dosing. Also approved for enlarged prostate symptoms at 5 mg. Not affected by food. Back pain and muscle ache in 4 to 6 percent |
| Sildenafil (Viagra, generic since 2017) | 30 to 60 minutes; half-life about 4 hours | 25 to 100 mg as needed | Cheapest and best studied. A high fat meal delays absorption by about an hour. Visual blue tint in 2 to 3 percent at 100 mg. Peak effect makes headache and flushing more noticeable |
| Vardenafil (Levitra, Staxyn) | 25 to 60 minutes; half-life 4 to 5 hours | 5 to 20 mg as needed | Similar to sildenafil in effect. Slightly more QT prolongation, so avoided with antiarrhythmic drugs. Less commonly prescribed now that the others are generic |
| Avanafil (Stendra) | 15 to 30 minutes; half-life about 5 hours | 50 to 200 mg as needed | Fastest onset and the most selective for PDE5, so fewer visual and muscle side effects. Still brand only in the United States and expensive |
Erectile dysfunction after 40 is a heart warning because the arteries that supply the penis are 1 to 2 mm wide and the coronary arteries are 3 to 4 mm wide, so the same plaque and the same endothelial damage produce symptoms in the smaller vessel first. Montorsi and colleagues in Milan found that among men who went on to develop both conditions, erectile dysfunction appeared 2 to 5 years before the first coronary symptom, a median of about 3 years. The Inman 2009 Mayo Clinic study of 1,402 men followed for 10 years found that in men aged 40 to 49, new erectile dysfunction carried a coronary disease incidence of 48.5 per 1,000 person-years compared with 0.94 in men the same age without it, a roughly 50-fold difference that shrank with age as heart disease became common in everyone.
The Massachusetts Male Aging Study established the scale of the problem in 1994: 52 percent of men aged 40 to 70 reported some degree of erectile dysfunction, and complete dysfunction rose from about 5 percent at 40 to 15 percent at 70. The point for a man over 40 is that a tadalafil prescription should come with a lipid panel, an ApoB, a blood pressure reading, an HbA1c and a morning testosterone, because the tablet treats the symptom while those tests find the cause. A 45-year-old whose erections have faded over two years and who has never had a calcium score is a man who needs one.
Daily tadalafil works about as well as on-demand tadalafil for erectile function scores, and better for spontaneity and for men who have sex more than twice a week. In the Porst 2006 trial, 268 men randomized to tadalafil 2.5 mg, 5 mg or placebo once daily for 12 weeks improved their IIEF erectile function domain score by 7.0 and 9.1 points respectively above placebo, and the share of successful intercourse attempts rose to 67 and 71 percent versus 31 percent on placebo. Rajfer 2007 replicated the result in 287 men. Head-to-head, the Buvat 2013 comparison of 5 mg daily against 20 mg on demand found no meaningful difference in erectile function scores at 12 weeks, with the daily group reporting higher satisfaction with spontaneity and the on-demand group reporting fewer tablets taken.
Two features of daily dosing matter for a man over 40 specifically. The first is that the effect holds over time: a 2-year open-label extension of the daily trials found no loss of efficacy and no evidence that PDE5 receptors adapt to constant exposure, which addresses the common worry that a man will become dependent or that the drug will stop working. The second is that daily tadalafil is the only PDE5 regimen with randomized data for penile rehabilitation after prostate surgery, where the goal is to keep the erectile tissue oxygenated during nerve recovery. The evidence there is mixed, but the logic, that regular blood flow protects the smooth muscle from fibrosis, is the same logic that makes daily dosing attractive to men whose erections are slowly declining rather than gone.
Daily tadalafil 5 mg reduces the urinary symptoms of an enlarged prostate by 4 to 6 points on the International Prostate Symptom Score, an effect that appears within 4 weeks and matches the alpha blocker tamsulosin, which is why the FDA approved it for benign prostatic hyperplasia in October 2011. The four registration trials pooled by Porst in 2011 covered 1,500 men with moderate to severe symptoms; tadalafil 5 mg cut IPSS by 4.8 to 6.3 points from baseline against 2.2 to 3.6 on placebo, with the same improvement in men with and without erectile dysfunction. The mechanism is that PDE5 is abundant in the smooth muscle of the prostate, bladder neck and the arteries supplying both, and relaxing it eases the outflow obstruction and improves the blood supply to a chronically under-perfused gland.
For a man over 45 who is up twice a night and whose erections have also faded, tadalafil 5 mg addresses both with one tablet and none of the retrograde ejaculation or dizziness that tamsulosin can cause. Unlike alpha blockers, tadalafil does not change urinary flow rate much on a flow test; the benefit is in symptoms and in bother score rather than in the peak flow number. It does not shrink the prostate the way finasteride does, so a man with a very large gland, above 40 to 50 mL on ultrasound, usually needs a 5-alpha reductase inhibitor as well. The Casabé 2014 trial found that adding tadalafil 5 mg to finasteride improved symptoms within 4 weeks, months before finasteride alone reaches its full effect, which is a reasonable way to bridge the gap.
Tadalafil improves the function of the lining of the arteries within weeks, and men prescribed PDE5 inhibitors after a heart attack have lower mortality in large registries, though the second finding is observational and no randomized trial has yet tested a PDE5 inhibitor as a heart drug in men without pulmonary hypertension. Rosano and colleagues in 2005 gave 32 men with cardiovascular risk factors tadalafil 20 mg every other day for 4 weeks and measured flow-mediated dilation of the brachial artery, the standard test of endothelial function. FMD improved significantly on tadalafil and the improvement was still present 2 weeks after the last dose, which suggests the drug does something to the vessel wall rather than simply dilating it while present. Aversa in 2008 found the same in men with type 2 diabetes using daily 5 mg.
The outcome data come from Sweden and the United States. Andersson 2017 in Heart followed 43,145 Swedish men after a first myocardial infarction and found that the 7 percent who filled PDE5 inhibitor prescriptions had 33 percent lower all-cause mortality and 40 percent fewer heart failure hospitalisations, with a dose-response: men who filled more prescriptions did better. A 2021 follow-up in the Journal of the American College of Cardiology compared PDE5 inhibitors with alprostadil, a different erectile dysfunction treatment, in 18,542 men with stable coronary disease, to reduce the chance that the benefit came from simply being healthy enough to want treatment; the PDE5 inhibitor group still had 12 percent lower mortality. Kloner 2023 in the Journal of Sexual Medicine looked at 72,498 American men with erectile dysfunction and found PDE5 inhibitor users had 13 percent fewer major cardiac events and 39 percent lower all-cause mortality. Healthy-user bias explains part of that gap. The consistency across three cohorts, the dose-response and the mechanism make it unlikely to explain all of it.
The Heart Data Is Encouraging But Not A Prescription
No man should take daily tadalafil to protect his heart on the strength of registry studies, and no cardiologist prescribes it for that purpose. What the data do establish is that a PDE5 inhibitor is safe in men with stable coronary disease who are not on nitrates, that it improves the function of the artery lining, and that men who take one after a heart attack are not putting themselves at risk. For a man over 40 with erectile dysfunction, that removes the main reason many men avoid the drug, which is a vague fear that it is bad for the heart. The evidence points the other way.
Tadalafil does not lower testosterone, and PDE5 inhibitors may raise it slightly, though the effect is small and probably indirect. In the Spitzer 2012 trial in Annals of Internal Medicine, 140 men with erectile dysfunction and low testosterone took sildenafil for a run-in period and then were randomized to add testosterone gel or placebo for 14 weeks. Testosterone rose modestly during the sildenafil-only run-in, and adding testosterone gel afterwards produced no further improvement in erectile function over sildenafil alone. The likely explanation for the small rise is that more frequent sexual activity and better mood both nudge testosterone up, rather than a direct effect on the testicle. Either way, the direction is reassuring for a man worried about hormones.
The muscle and exercise data are thin but interesting. Sheffield-Moore and colleagues in 2013 gave older adults sildenafil 25 mg three times a day for a week and measured a 30 percent increase in skeletal muscle protein synthesis and reduced muscle fatigue, a result that has not been followed up with a training study. At sea level, PDE5 inhibitors do not improve endurance performance in healthy men; at simulated altitude of 3,874 metres, Hsu 2006 found sildenafil improved cycling time trial performance by about 15 percent in the subset of men who were strong responders, because the drug lowers pulmonary artery pressure. The World Anti-Doping Agency reviewed PDE5 inhibitors and did not add them to the prohibited list. For a man over 40 who lifts, tadalafil is neither a performance drug nor a performance risk, and the one training-relevant side effect is the back and muscle ache that 4 to 6 percent of men get in the first weeks, which is usually dose-dependent and settles.
Any man taking a nitrate medication in any form must not take tadalafil, because the combination can drop blood pressure far enough to cause collapse or a heart attack; that includes nitroglycerin tablets and sprays, isosorbide, and recreational amyl or butyl nitrite poppers. The interaction is not dose-dependent and the effect of tadalafil lasts up to 36 hours after a single 20 mg tablet, so a man who might be given nitroglycerin in an emergency needs to tell the emergency team when he last took it. The other contraindications are a heart attack or stroke within the past 6 months, unstable angina, severe heart failure, uncontrolled blood pressure above 170/100 or below 90/50, severe liver disease, and a history of non-arteritic ischaemic optic neuropathy, a rare form of sudden vision loss that has been reported in about 3 per 100,000 PDE5 inhibitor users a year, mostly in men with a small optic disc who already had vascular risk factors.
Alpha blockers for the prostate or for blood pressure, such as tamsulosin, doxazosin and terazosin, are not a contraindication but need care: both drugs dilate vessels, and starting tadalafil in a man already on doxazosin can cause dizziness on standing. Tamsulosin 0.4 mg with tadalafil 5 mg daily has been tested and is well tolerated. Drugs that block the CYP3A4 liver enzyme, including ketoconazole, ritonavir and clarithromycin, raise tadalafil levels several-fold and the daily dose should be halved or held. Grapefruit juice has a small version of the same effect. Priapism, an erection lasting more than 4 hours, is rare with PDE5 inhibitors alone but more likely in men also using injectable alprostadil or a melanocortin peptide, and needs emergency treatment.
Tadalafil Interactions And Cautions For Men Over 40
| Drug Or Condition | Status | What To Do |
|---|---|---|
| Nitrates: nitroglycerin, isosorbide, nicorandil, poppers | Absolute contraindication | Never combine. If nitroglycerin is needed in an emergency, at least 48 hours must have passed since the last tadalafil dose. Tell any emergency clinician about tadalafil use |
| Riociguat (for pulmonary hypertension) | Absolute contraindication | Same mechanism of dangerous blood pressure drop. Not combined under any circumstances |
| Alpha blockers: tamsulosin, doxazosin, terazosin, alfuzosin | Caution | Stabilise on the alpha blocker first, then start tadalafil at 2.5 mg. Tamsulosin 0.4 mg with tadalafil 5 mg is the best studied and best tolerated pairing |
| Strong CYP3A4 inhibitors: ketoconazole, itraconazole, ritonavir, clarithromycin | Caution | Tadalafil levels rise 2 to 4 fold. Daily dose capped at 2.5 mg or held during a short course of the interacting drug |
| Heart attack or stroke in the past 6 months, unstable angina | Contraindicated until stable | Wait for cardiology clearance. Once stable, PDE5 inhibitors are safe in coronary disease and the registry data are favourable |
| Blood pressure above 170/100 or below 90/50 | Hold until controlled | Treat the blood pressure first. Well controlled hypertension on any non-nitrate drug is not a barrier |
| Testosterone therapy, GLP-1 drugs, statins, finasteride, metformin | No interaction | Commonly combined in men over 40. Tadalafil does not affect the dosing of any of them |
| Severe kidney disease (eGFR under 30) or moderate to severe liver disease | Dose reduction | Daily dosing is not recommended below eGFR 30; on-demand 5 mg with a 72 hour gap. Avoid in Child-Pugh C liver disease |
Tadalafil and testosterone therapy address different parts of the same problem and are frequently combined in men over 40: testosterone restores desire and the nerve and tissue signalling that erections depend on, while tadalafil amplifies the blood flow response to that signal. A man with a morning testosterone under 300 ng/dL and no desire will often get a weak result from tadalafil alone, because the drug needs a stimulus to work with. Buvat 2011 in the Journal of Sexual Medicine tested this directly: 173 men who had not responded to tadalafil were given testosterone gel or placebo alongside it for 12 weeks, and the men whose baseline testosterone was under 300 ng/dL improved significantly on the combination, while men with normal testosterone gained nothing from the added hormone.
The reverse is also common. A man on well-managed testosterone therapy whose levels are in the 600 to 800 ng/dL range and whose desire has returned may still find erections unreliable, because two decades of high blood pressure, insulin resistance or smoking have damaged the arteries in a way testosterone does not repair. That man is the ideal daily tadalafil candidate, and the two together are one of the most reliable combinations in men's health. What neither drug fixes is the underlying vascular disease, so the same lipids, blood pressure, waist and sleep that determine his testosterone determine how long tadalafil keeps working.
Step 1
Test before treating
Morning total and free testosterone with SHBG, lipids and ApoB, HbA1c and fasting insulin, blood pressure, and a PSA if over 45. Erectile dysfunction is a cardiovascular finding and the workup should reflect that
Step 2
Testosterone normal, desire intact
Erections are the problem, not drive. Daily tadalafil 5 mg or on-demand 10 to 20 mg depending on frequency and preference. Address blood pressure, lipids and waist alongside
Step 3
Testosterone under 300, desire low
Start with testosterone therapy or, in a man who wants to preserve fertility, enclomiphene or hCG. Add tadalafil after 8 to 12 weeks if erections have not followed desire back
Step 4
Testosterone under 300, tadalafil already failed
The Buvat 2011 pattern. Restore testosterone and continue tadalafil; most men in this group respond to the combination that failed on tadalafil alone
Step 5
Both optimised, still unreliable
Check the tadalafil is being taken daily and not just on the day, screen for sleep apnea and depression, and consider a penile Doppler ultrasound to quantify arterial inflow. Second line options include a melanocortin peptide or intracavernosal injection
Week 0: Baseline and screening
Weeks 1 to 2: Start at 2.5 or 5 mg each morning
Weeks 3 to 4: First assessment
Weeks 5 to 8: Adjust or add
Weeks 9 to 12: Settle the regimen
The Tablet Treats The Symptom; The Bloodwork Finds The Cause
A man over 40 who is started on daily tadalafil without a lipid panel, a blood pressure reading, an HbA1c and a testosterone has been given the right drug by the wrong process. Erectile dysfunction is the earliest visible sign of arterial disease in most men, arriving about three years before the coronary version, and the tablet works well enough that the warning can be silenced without the cause being found. Tadalafil is a good drug. It is a better drug when it comes with a calcium score.
Daily tadalafil 5 mg turns a drug built for an evening into one that acts on the arteries, the prostate and the erectile tissue every day. The randomized trials show it improves erectile function scores by 7 to 9 points, matches on-demand 20 mg for effect while beating it for spontaneity, cuts prostate symptom scores by 4 to 6 points within a month and improves the function of the artery lining within 4 weeks. It does not lower testosterone, it combines cleanly with testosterone therapy, GLP-1 drugs and statins, and the registry data on heart outcomes point in a favourable direction. The one rule that cannot be bent is the nitrate rule. And the one habit that turns a good prescription into a good decision is drawing the lipids, the blood pressure, the HbA1c and the testosterone before the first tablet, because erectile dysfunction at 45 is the earliest sign of the disease that will otherwise show up in the coronary arteries three years later.
